New Prospective for Reversal Strategies in Bleedings on Patients Assuming Direct Oral Anticoagulants

چکیده
Direct oral anticoagulants (DOAC) are an effective and safe therapeutic option for venous thromboembolism or nonvalvular atrial fibrillation (NVAF). Four trials on non-valvular atrial fibrillation patients (RE-LY for dabigatran, ROCKET-AF for rivaroxaban, ARISTOTLE for apixaban, and the ENGAGE-AF for edoxaban) evaluated the efficacy and safety of DOACs in comparison to vitamin K antagonists (VKAs), showing a lower incidence of intracranial haemorrhage and fatal bleeding [1-6].

Tutorial in oral antithrombotic therapy: Biology and dental implications
OBJECTIVES Recent developments of new direct oral anticoagulants that target specific clotting factors necessitate understanding of coagulation biology. The objective of this tutorial is to offer dental professionals a review of coagulation mechanisms and the pharmacodynamics of the conventional and new oral anticoagulants. Also, we summarized the dental implications of the conventional and new...

To date, clinical studies show that the incidence of spontaneous bleeding with new direct oral anticoagulants (DOAs) is comparable to that of established anticoagulants. However, unlike vitamin K antagonists, there are currently no clinically available antidotes or approved reversal agents for new DOAs. Restoring normal coagulation is important in many cases, such as emergency surgeries, seriou...

PURPOSE Strategies for the management of bleeding complications and facilitation of an invasive procedure in patients receiving direct oral anticoagulants (DOACs) are reviewed. SUMMARY The DOACs provide clinical advantages versus vitamin K antagonists, including fixed dosing with no routine coagulation monitoring and evidence of a lower risk of bleeding. However, as with all anticoagulants, t...

BACKGROUND Recently, a new generation of direct-acting oral anticoagulants (DOACs) with a greater specificity towards activated coagulation factors was introduced based on encouraging results for efficacy and safety in clinical studies. An initial limitation of these new drugs was the absence of an adequate strategy to reverse the effect if a bleeding event occurs or an urgent invasive procedur...
متن کاملMinimizing bleeding risk in patients receiving direct oral anticoagulants for stroke prevention
Many primary care physicians are wary about using direct oral anticoagulants (DOACs) in patients with nonvalvular atrial fibrillation (AF). Factors such as comorbidities, concomitant medications, and alcohol misuse increase concerns over bleeding risk, especially in elderly and frail patients with AF. This article discusses strategies to minimize the risk of major bleeding events in patients wi...
